From Homelessness to Independence

Stage 1
The Problem
A young person aged 17 was referred to us by their local authority following a placement breakdown. They had been sofa surfing for several weeks, had no access to education or employment, and were at significant risk of long term homelessness. Their confidence was very low and they had experienced multiple placement disruptions in the past, making them understandably cautious about engaging with support services.
Stage 2
The Intervention
Within 48 hours of referral we completed a full needs assessment and provided a safe, stable room in one of our supported properties. A named Key Worker was assigned who met with the young person three times a week in the early weeks to build trust and understand their goals. Together they developed an individualised support plan focused on three areas: emotional wellbeing, reconnecting with education, and developing practical life skills including budgeting and cooking. We also facilitated referrals to local mental health services and worked closely with the placing authority throughout.
Stage 3
The Outcome
Within four months the young person had enrolled on a Level 2 qualification at a local college, was managing their own budget independently, and had developed a strong support network within the service. By the time they turned 18 they were ready to transition into semi independent living, which they did successfully. Their social worker described the transformation as remarkable. The young person told us they finally felt like they had a future worth working towards.
